Application for Reinstatement to Amateur Status
A golfer who has breached the Rules of Amateur Status and wishes to be reinstated as an amateur golfer must apply for reinstatement. A candidate must download the applicable application form above and submit it to the appropriate provincial golf association who will then forward it to the Canadian Professional Golfers' Association. The CPGA will then submit it to the RCGA to be processed. Former or resigning members of the Canadian Professional Golfers' Association may submit the application directly to the CPGA in Acton, Ontario.
